Physical Therapist

Job Responsibilities:
The physical therapist has the responsibility to restore patient's function, alleviate pain and prevent further disabilities by planning and administering medically prescribed physical therapy.

Job Qualifications:
Essential Job Functions/Responsibilities: 1. Provides physical therapy services to patients according to physician orders. This may include, but will not be limited to: • Assessing and evaluating therapeutic/rehabilitative/functional status and participation in the development of the care plan. Evaluate home environment and design home exercise program. • Direct physical therapy treatment and instruct patients, families and caregivers in the home exercise program • Recommends and/or provides assistive equipment for use in the patient's home. • Determining priority needs for physical therapy • Administers manual exercises; instructing, encouraging and assisting patients in performing physical activities, ambulatory functional activities and daily living activities. • Evaluates effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting and evaluating patient's progress; recommending adjustments and modifications • Identifies patient and family/caregiver needs for other home health services and refers as necessary • Instructs other personnel and/or family/caregiver of areas of treatment that they may assist patient, as well as instructing them on the goals for the patient. • Prepares and submits clinical and progress summaries based on the attainment of goals. • Participates in discharge planning for the patient • Provides in-service education programs for home health personnel as needed • Supervises physical therapy assistants every 30 days when re-evaluation of patient is done. • When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every 2 weeks

Position Qualifications: 1. Graduate of an approved Baccalaureate or Masters program in physical therapy. 2. Licensed to practice as a physical therapist in Kansas 3. Experience as a physical therapist in community/home health preferred 4. Demonstrates good verbal and written communication and organizational skills 5. Possess and maintain current CPR certification
